Aquagenic Urticaria: Aquagenic urticaria is a rare type of physical urticaria in which contact with water causes itching and small hives on the skin. It can occur with any type of water, whether it is cold, warm, tap water, sweat, or even tears. When water comes in contact with the skin, it interacts with substances present on or within the skin, leading to release of histamine from mast cells. This histamine release causes itching, redness, and formation of small wheals or hives. Symptoms usually appear within a few minutes of water exposure and may last for 30 to 60 minutes. The rash is typically seen on the neck, trunk, and upper limbs, while palms and soles are usually spared. This condition is not due to allergy to water itself, but due to an abnormal skin reaction triggered by water contact. Diagnosis is done by a water challenge test, where water is applied to the skin and observed for reaction. Management includes use of antihistamines to reduce symptoms, and in some cases barrier creams or phototherapy may be helpful. Avoiding prolonged water exposure can also reduce symptoms.
