"[18F]FDG PET/CT is a high-yield diagnostic tool that directly influenced clinical management of approximately 75% of patients with FUO, with optimal performance in patients with elevated inflammatory markers, fever at the time of imaging, and limited prior antibiotic use." jnm.snmjournals.org/content/early/…

📷 Case of the Day: Krukenberg Tumor on FDG PET/CT A patient presents with FDG-avid bilateral ovarian masses on PET/CT. 📷 The key question: Primary ovarian tumor or metastatic disease? Krukenberg tumors are metastatic ovarian tumors, most commonly arising from gastric carcinoma, and often demonstrate intense FDG uptake on PET imaging. On FDG PET/CT, we may see: • 📷 FDG-avid ovarian metastases (often bilateral) • 📷 FDG-avid gastric primary tumor • 📷 Peritoneal or nodal metastatic disease A great reminder of how molecular imaging helps uncover the full story behind ovarian masses. 📷 Sharing this interesting case. #fdgpet #NuclearMedicine #MolecularImaging #radiology #meded

📷📷 Case Spotlight: “Superscan” on PSMA PET 📷 PSMA PET Findings: Diffuse, intense tracer uptake throughout the skeleton with markedly reduced soft tissue and visceral activity. 📷 Takeaway: Classic PSMA “superscan” → indicates extensive skeletal metastatic disease (prostate cancer). 📷 Pearl: Reduced renal/soft tissue visualization occurs due to tumor sink effect. #NuclearMedicine #PSMAPET #ProstateCancer #Radiology #CaseOfTheDay #NuclearMD

🧠⚛️ Case Spotlight: Lingual Thyroid 🔬 Imaging Findings: • Iodine Scan (I-123 / Tc-99m pertechnetate): 👉 Focal radiotracer uptake at the base of the tongue 👉 Absence of normal thyroid uptake in the neck • CT Scan: 👉 Well-defined, hyperdense enhancing mass at the base of tongue 👉 No orthotopic thyroid gland identified in the thyroid bed 💡 Diagnosis: Lingual Thyroid (ectopic thyroid tissue) ⚠️ Why it matters: • Often the only functioning thyroid tissue • Biopsy or excision without confirmation can lead to hypothyroidism • Can present with airway or swallowing symptoms 🎯 Key Teaching Point: Always confirm with radionuclide imaging before any intervention on a midline tongue-base mass!
