In its recent report, Guidance for Best Practices for Clinical Trials, the @WHO advocates for the use of Core Outcome Sets in clinical trials. Core Outcome Sets represent a minimum set of outcomes that must be measured in all trials, ensuring consistency and standardization across studies. Adoption of Core Outcome Sets: 🔹 Ensures outcomes important to patients and decision-makers are used in clinical trials. 🔹 Reduces research waste by having the same meaningful data across trials. 🔹 Promotes harmonization of clinical trials, thus fostering stronger evidence synthesis to inform policy and practice. The Red Hat Group (RHG) fully supports the World Health Organization's recommendation, recognizing the transformative potential of Core Outcome Sets to advance the quality and impact of clinical research. By advocating for the widespread adoption of Core Outcome Sets, RHG seeks to align clinical research with the needs of patients and healthcare providers, fostering collaborative development, shared knowledge, and improved outcomes across trials. Through the RHG, our organizations remain committed to promoting scientific collaboration and facilitating the adoption of Core Outcome Sets through joint initiatives that bridge the gap between research, health policy and clinical practice. OMERACT Working Group members (@DrRChristensen & Marissa Lassere) have contributed to the development of both the SPIRIT-Surrogate and CONSORT-Surrogate outcome guidelines, aimed at enhancing the rigour and transparency of trials that employ surrogate endpoints. Randomized controlled trials frequently adopt surrogate endpoints to efficiently approximate target outcomes of primary interest to patients, clinicians, and other stakeholders (e.g., all-cause mortality), offering benefits such as shorter follow-up periods, reduced sample sizes, and lower costs. Despite these efficiencies, surrogate endpoints pose inherent limitations—primarily, uncertainty regarding the intervention’s true effect on the target outcome and incomplete information about potential harms.
