The physical exam is a lens for the diagnostic test. It focuses you to the specific level or spot where the lesion likely lies. The radiologist lacks the exam. I recently caught a missed C7 lesion, that I might have missed too if not for me knowing the triceps reflex was brisk.

At our last exam, this one person just broke down. "Why should I bloody care if the plantars are upgoing or downgoing? Or if there is an upper limb weakness first or lower limb weakness first? If a patient comes to me with neck pain and left arm weakness, I am going to order an MRI, if the patient comes with neck pain and right arm weakness, I am going to order an MRI. And don't tell me there is a situation where there is no MRI. What am I going to do? Randomly open up the patient's cervical spine and operate?"
