Tumor Tip of the Week | EOD Pancreas – Scan vs Path Pancreas head – IPMN with invasive carcinoma. CT scan showed a 6.2 cm cystic lesion. Main pancreatic duct dilated to 18 mm. Veins and artery were abutted. Looks scary on imaging. Surgery - Whipple resection. Pathology told the real story: • Tumor reached duodenal wall and peripancreatic tissue • Invasive carcinoma at pancreatic neck margin • All 23 lymph nodes were negative • No lymphovascular or perineural invasion Code EOD 500 – Pathology rules. Vein abutment on CT does not count unless confirmed by pathology. Always let pathology guide EOD coding.
