But then something changed. On the right side, the scene becomes dense… pink… heavy. This is where the blood supply was suddenly cut off. A retroplacental hemorrhage lifted the placenta away from the uterine wall. The maternal blood stopped bathing those villi. The islands were stranded. Without oxygen, the cells began to die. Under the microscope, death looks like this: •The villi lose their delicate detail. •The nuclei fade and disappear. •The tissue becomes solid and glassy pink. •The structure is still there — but it’s a “ghost” of what it was. That big dense pink region? That is an infarct — a graveyard of once-living villi. And along the bottom, you can even see darker red hemorrhage — the reminder of the original bleeding event.

Before reaching the climax of the story of this disease ,normal histology should be explained. The placenta is made of tiny structures called chorionic villi. Each villus floats in maternal blood like a small island. The outer layer (trophoblast) touches the mother’s blood. Inside the villus are small vessels carrying the baby’s blood. Mother’s blood stays outside. Baby’s blood stays inside. They never mix — but oxygen and nutrients pass across a very thin barrier. This thin area, where fetal capillaries come very close to the surface, is what allows efficient gas exchange. So normally, these villi are delicate, well-vascularized, and perfectly designed for oxygen transfer. Now imagine what happens when blood suddenly collects behind the placenta and cuts off that maternal flow… 👀 In placental abruption, blood collects behind the placenta (retroplacental hematoma). The placenta lifts off the uterine wall. Maternal blood can no longer properly bathe the villi. Those tiny exchange “islands” lose their oxygen source. Ischemia begins. If prolonged → the villi die. That death of villi due to lack of maternal blood flow is called: Villous infarction. 🔬 What does infarction look like?

(Histopathology): Elongated, bulbous rete ridges forming a reticular pattern (less prominent on the face), with marked basal layer hyperpigmentation and a mild increase in melanocytes, especially at the tips.

Answer is solar lentigo: Solar lentigines are benign lesions occurring on sun-exposed skin. Pathogenesis is related to repeated intermittent sun exposure and ultraviolet-induced mutations leading to enhanced melanin production and abnormal pigment retention by keratinocytes.
