What is really funny I received my first passport USING A HOSPITAL Birth Form. IT DID NOT WORK FOR THE RENEWAL SO (I actually thought it was a birth certificate because it worked the first time) I called and got one in about 5 minutes. Now on 3rd passport and Middle name is not the same on any of the three, Ya know Confirmation name becomes prevelant. But because you use Birth Cert to get passport My newest one came with a form stating it has different middle name or initial for one of these 5 reasons. That made me Laugh! Yup It's me. By the way I could not get a real ID because my Birth Certificate has a different middle name than my Driver license etc??? Weird!
