Half of Black people diagnosed with HIV last year were diagnosed too late. Late diagnosis has real consequences. It means people miss out on early treatment, face a higher risk of serious illness or death, and may unknowingly pass HIV on to partners because they are not yet on treatment. HIV is treatable. Treatment saves lives. Treatment stops transmission. We need to remove these barriers. Self-testing and self-sampling allow people to test privately, easily, and without shame - and link straight into care when needed. Early testing saves lives. Access matters. PrEP is one of the most effective HIV prevention tools we have, offering near 100% protection yet uptake remains unequal. Dr Michael Brady, Sexual Health & HIV Consultant at King’s College Hospital, highlights why Black communities are less likely to access PrEP: limited awareness, stigma, and the fact that PrEP is still largely confined to sexual health clinics. The rollout of injectable PrEP, administered every two months, presents an opportunity to remove some of these barriers particularly for Black and other underserved communities. But innovation alone is not enough. PrEP must be understood as a tool for anyone at risk of HIV, not a niche intervention. Healthcare professionals outside of sexual health also need better education so conversations about PrEP are happening in GP practices, pharmacies, and community settings - not just specialist clinics. World AIDS Day isn’t a ceremonial date, it’s a wake-up call. As @ondeni makes clear in Episode 6 of our monthly broadcast, the fight is carried every day by community organisations, peer advisors and activists who fill the gaps that systems continue to ignore. We already have the tools to end HIV with PrEP, U=U, testing, treatment - yet inequalities keep the epidemic alive. If we’re serious about ending HIV, we need to invest in the people doing the real work on the ground. Visibility isn’t enough - sustained support and funding are what will actually shift the needle.
