Most persistent low back pain patients have already tried exercises. But many still struggle with sitting, bending, walking or lifting. So the real question becomes: How do you help them regain control of those movements again? One of the biggest challenges with persistent back pain is that symptoms often show up in specific daily tasks. Sitting too long. Bending forward. Getting up from a chair. Picking something up. If rehab stays too general, patients often leave the clinic stronger… …but still unsure how to move when pain shows up. In this Practical, we walk through how to use behavioural experiments inside your assessment and treatment. A few key ideas: • Test patient-specific tasks Assess movements that actually trigger symptoms. Sitting, sit-to-stand, bending, walking or lifting. • Use behavioural experiments Small modifications during the movement help identify what changes symptoms. • Let patients see their own data People rarely argue with what they feel themselves. • Build self-efficacy The goal isn’t just reducing pain. Most hamstring injuries are still labelled Grade 1–3. The problem? Two injuries can both be called “Grade 2” and behave completely differently depending on location and tendon involvement.
