This a remarkable story of medical reversal that every doctor should read. High dose dexamethasone was a standard component of chemo for multiple myeloma (I remember this from residency in the 1990s) until this practice was challenged by a patient advocate and subjected to an RCT which showed it was very harmful. Echoes of the CAST trial for antiarrhythmic drugs. In both cases, experienced clinicians were unable to see harm in practice without the clarity of RCTs. We should always be questioning what medical practices now accepted as "standard of care" are actually harmful. @adamcifu @drjohnm @VPrasadMDMPH @AndrewFoy82

I would like to share the story of how a patient with cancer came up with the idea for a randomized trial, & how listening to him saved a lot of lives. 1/ In 2002, I had just completed a randomized trial with the notorious drug thalidomide for the cancer, multiple myeloma.

Another reported series of patients with spinal CSF-venous fistulas treated by endovascular embolization expands the data available supporting this novel treatment paradigm. Schreiber et al. report a series of 21 patients undergoing 23 procedures with 86% of patients experiencing both clinical and radiographic resolution of their symptoms of spontaneous intracranial hypotension. How accurate is assessment of extrinsic vs. intrinsic venous sinus stenosis on MRV? Obviously intrinsic after stent deployment in this case. Venous angioplasty is then a distinct technique, with serial upsizing of noncompliant balloons. @iih_hub @Tinnitus_USA @SNISinfo
