The p.Val142Ile (Val122Ile) variant is the most common hereditary ATTR amyloidosis mutation in the U.S., disproportionately affecting individuals of African ancestry. This review highlights ongoing challenges in timely diagnosis, genetic screening, access to specialty care, and equitable treatment, while outlining priorities to improve outcomes through earlier identification, multidisciplinary care, and more inclusive clinical trials. A timely and important read (link.springer.com/article/10.100…) #HREV @senthil_selv @KMAlexanderMD @NotNoninvasive

This is going to be an outstanding experience!!! So proud and happy of my alma mater! As a general fellow the procedural volume was amazing. 317 coronary angiogram and PCI cases including IVUS and FFR/iFR (within just 4 months), > 10 pericardiocentesis, > 50 RHC, > 20 endomyocardial biopsies (shoutout to Dr. Svetlichnaya), > 15 IABPs, 8 PFO closures (shoutout out to Dr Romero) etc. This is going to be even better for the interventional fellows. Apply!

U-CARS 2026 highlights: refined β-adrenergic signaling enabling more targeted β-blocker strategies, intercellular crosstalk driving remodeling, and HF as a multisystem (cardio-renal-metabolic) syndrome—pointing toward integrated, precision therapy (link.springer.com/article/10.100…). #HREV

VO₂max remains the benchmark for CPET in HF, but it often misses subtle functional impairment. Submaximal metrics—VAT, VE/VCO₂ slope, OUES, EOV, O₂ pulse, COP, and VO₂ kinetics—provide powerful prognostic insight and individualized care plans (link.springer.com/article/10.100…). #HREV

Arrhythmias remain a major challenge in patients supported with LVADs. This state-of-the-art review highlights the high burden of AAs/VAs, the impact on RV failure, limitations of current therapies, and the need for close HF–EP collaboration (link.springer.com/article/10.100…). #HREV

@KiranKhush1, @brian_wayda, and colleagues demonstrate that racial disparities in outcomes after OHT persist in the contemporary era over the long term. Black patients have more adverse post-OHT events, such as rejection, new onset DM + worsening renal function. Biological differences and susceptibility likely also play a role. Congrats to @ankeetbhatt for KP VACCINATE LBCT @ESC2025. Largest RCT ever conducted with >3.6 million patients randomized in 1 month. No effect on vaccination rates but proof-of-concept for large-scale embedded RCT within a LHS!

A new concept from 2025 ESC guidelines on myocarditis and pericarditis presented: the IMPS (Inflammatory MyoPericardial Syndrome) acronym. A new term to describe the full spectrum of myocardial and pericardial inflammatory diseases and improve the management of patients.

HFpEF is marked by elevated LV filling pressures due to diastolic dysfunction. This review explores how intrathoracic volume & pressure—affected by posture, gravity, obesity, & exercise—shape cardiac filling & highlight research gaps (link.springer.com/article/10.100…). @JLHarrington_MD
