Nice little APP referral from an attending ambulance crew, where I was able to supply the patient with a course of medication (using PGD) and send a clinical summary their GP using @SECAmb_IBIS to inform of follow-up arrangements 👌🏻 #AdmissionAvoidance #RightCareRightTime

Our Falls Risk Assessment Tool (FRAT) triages a patient’s risk of further falls, sending an electronic 🔴 P1 referral 🟠 P2 referral or 🟢 P3 notification to a local Community Falls Service and/or the patient’s GP 🏠📲➡️🧑🏻‍⚕️

@emily_restell @MarcusYalman @Ben_Roberts24 I think there needs to be wider education- I’m in sussex and used to respect doc (generally in depth) and EMIS records via @SECAmb_IBIS but if I end up in Kent I have to get my head round TEP forms which knocks confidence. A place for a wider systems?

This includes 18,500 DNACPR and 7,600 ReSPECT. However increasingly sensible clinical decisions are being made through discussions with family 👍🏻

Yesterday, @Ldn_Ambulance called to a dying breathless lady, no form as was away from home, listened to story from daughter, waited and comforted till last breath. No CPR. All down to training by @DianeLaverty4 & co

A new time EVER record... In the eight years that @SECAmb_IBIS has been used within the Trust, this month almost 1️⃣0️⃣% of patients who called 999 had a care plan available to support care! 😃 #AdvanceCarePlanning #PersonCentredCare
