I have a patient who has ejection systolic murmur, that was so hard to pick even had to do Echo to confirm finding. Today I was supervising a medical student, and on mere cardiac examination he picked that murmur up. Mere saath saath consultant ki bhi phat gai aik dfa tou 😂

"So which of the favors of your Lord would you deny?" (55:13) ✨ Incoming Infectious Diseases Fellow – Westchester Medical Center, NY 🇺🇲 It took me five years. “I don’t have the resources. Can I pursue the USMLE journey?” I asked a senior during my fourth year of MBBS, soon after he had matched. His response, “Only if you are willing to pay the price,” struck me deeply. ‘Could the son of a laborer, from a family surviving on a few thousand rupees a month, really dream this big? Was this path not reserved only for the privileged?’ For years, such questions consumed me. But not anymore. From that day on, I did not stop. Yes, it took me five years. Balancing work as a medical instructor and medical officer, long residency hours, and a full-time moonlighting job, all while surviving on four to five hours of sleep, was never easy. The journey was full of uncertainty, visa rejections, moments of humiliation, and countless tests of patience. Yet I kept going with quiet defiance, smiling through it all, grounded in discipline, consistency, and faith. Since the early days of medical school, the United States has always been my goal. Despite initial hesitation, financial burdens, and the knowledge that my path would take longer than most, making me an older graduate and placing me at a disadvantage, I still chose to begin. I could not allow my circumstances to define me, nor could I live with the regret of not even trying if my conscience were to question me in my forties or fifties, because Allah says, “That man can have nothing but what he strives for.” (53:39) I would be remiss if I did not acknowledge the people who lifted me when I needed it most, as none of this would have been possible without them. This success belongs as much to my parents, family, mentors, colleagues, and friends as it does to me. To those who stood by me, thank you. And to those who didn’t, you too played your part. To anyone walking this path through uncertainty, self-doubt, or unanswered questions: trust in your work and in Allah’s plan. Write your own story, one that inspires someone else to keep going when they feel like giving up. And if you ever feel like quitting, remember you were not born to settle for less. Alhamdulillah, countless times! 🙏

Health systems that are way ahead and progressive, there's a reason. They know how to give respect and strength of backbone that are residents of health system. UK, NHS will never reach to such level if current policies continue to do so.

The UK is decades behind at best. Basic facilities that most of us don’t have as doctors: 1. An office space solely for the team 2. A clean area to work 3. At least one workstation per doctor 4. Available drinking water that is readily accessible 5. A window in a room to work in 6. A sofa 7. A working clock 8. Desk space @NHSEngland harps on about a 10 point plan like it’s some inconceivable win. Is it any wonder why no doctor takes you seriously @wesstreeting? At least treat us like human beings. Or are we not worthy of that either?

@drraja_ I have seen worst. No FY1 to SHOs are allowed to speak or take opinion from few departments, only registrars and above speak to them. I understand pressure on such departments but I think if Consultant has advised for their opinion then SHOs can ask for their 2 minutes review.

Doctors in training are encouraged to consult with senior colleagues when uncertainties arise. However, resident doctors may face reprimand for excessive contact with senior staff. This dynamic often stems from an environment of apprehension and detachment fostered by some senior doctors, creating a challenging cultural reality within medicine that is not adequately addressed. From my perspective, a senior physician who is consistently inaccessible to resident physicians' inquiries should not be entrusted with supervisory responsibilities over them.

I am sure if residents in UK knew about current situation earlier, even brits doctors would definitely give a thought leaving NHS. In Pakistan, I would suggest my colleagues to start clinical experience, residency training and simultaneously working on portfolio and exams.

I would advise my colleagues to go for USMLE, if not then Royal College membership exams for UK then 1 year NHS experience then better to leave to Australia/NZ. Situation has been changed drastically in last 2 years unexpectedly. A suggestion from stranded colleague.
