Today, Lincolnshire’s Transfer of Care Hub celebrates its first Birthday! Since the hub launched, it has supported the discharge of approximately 6000 patients, wherever possible to their homes and, where necessary, with care and support wrapped around them.

We're doing quite a bit on building relationships, (listening, empathy), clarity, motivation etc. - so hopefully that supports managers to avoid getting into poor performance situations but had a specific request. Hopefully see you live on a Friday soon (school run permitting!)
