ASCO this year has 5,000+ abstracts.
But ~24 will actually change practice.
Here is that map.
Data-grid is the shortest path through ASCO 2026:
12 disease areas, 24 critical readouts, 5 plenaries, and 2 (confirmed) misses already on the board.
Few things jump out immediately:
▪️Daraxonrasib gets the headline (13.2 vs 6.7 mo)
▫️Sarcoma gets a plenary as public science funds P3 that pharma would not.
▫️Lung cancer remains crowded: RET adjuvant, bispecific OS, post-osimertinib, next-gen EGFR.
▫️Breast cancer reminds us that not every big swing lands
By next week, only a handful of these cells will become new standards of care.
This is your cheat sheet to keep that score.

PFS2 as a surrogate for OS in breast cancer? 🧠
🆕 ESMO Open 2026 systematic review of 18 RCTs (n = 9,617)
🎯 Question: Can PFS2 reliably predict OS in breast cancer?
📊 Primary analysis
•r = 0.71
•R² = 0.51
→ ~51% of OS variability explained by PFS2
⚠️ Long-term OS validation:
•r = 0.55
•R² = 0.30
→ Signal weakens with maturity
🔬 Surrogacy improves only when:
•PFS2 maturity ≥55% → R² = 0.86
•OS information fraction ≥75% → R² = 0.87
But not consistent in long-term validation.
📌 Interestingly:
PFS1 showed slightly stronger association with OS
•r = 0.77
•R² = 0.60
💡 Takeaway:
PFS2 is context-dependent, not a robust universal surrogate.
Endpoint maturity matters. Clinical setting matters.
Be cautious before accepting PFS2 as a primary decision driver.

⚠️ RT + Targeted Therapy = Hidden Toxicity Risk?
New ESMO–ESTRO consensus on combining radiotherapy with:
🔹 EGFR inhibitors
🔹 ALK inhibitors
🔹 BRAF/MEK inhibitors
📄 Review in ESMO Open 2026
🟣 EGFR inhibitors
• ↑ Dermatitis & mucositis risk
• Thoracic RT + osimertinib → high pneumonitis risk
• Major adaptation recommended in most high-dose settings
🔵 ALK inhibitors
• Evidence limited
• Most scenarios → major adaptation advised
• CNS combinations need caution
🔴 BRAF/MEK inhibitors
• Clear ↑ skin toxicity
• High-dose skin RT → avoid combination
• Vemurafenib + skin RT → do NOT combine

• Pegfilgrastim timing matters
• 24h → higher early WBC spike
• 72h → ↓ bone pain
• 72h → more controlled WBC curve
• No loss of efficacy
Maybe 72 hours is the sweet spot 👇
