@MattZirwas Your graph shows almost no improvement in your proficiency as a doctor in 16 years of practice from 2010 to 2026. What’s going on with that?

Whether AI is a better doctor than me is the wrong question. The right question is this: Which is best? Me alone, me + AI, or AI alone? That sounds like a dumb question, but it's not and the answer isn't obvious or static. A year and a half ago, the answer was me alone. AI made me worse. Slowed me down, gave me information that I had to verify and could have found faster on my own. As of today, me + AI is WAY better than me alone. And that gap keeps getting bigger. Part of it is the AI getting better, part of it is me getting better at using the AI. The two compound. My colleagues who don't get this are starting to get left behind. At an absolute minimum, start using OpenEvidence or ChatGPT for Clinicians. Both are free. Much better - build your own harness for Claude. It's better than either and not hard. One approach is in the first reply. Some day AI alone will be better than me + AI. I'll just slow it down, send it in wrong directions, decide I know better than it when I don't. But today isn't that day.

Jesus fucking christ doctors and mid-levels. Stop telling your patients the GLP1's are going to be covered under the bridge program, IT STILL REQUIRES A PRIOR AUTH THAT YOU NEED TO FILL OUT, ITS NOT AUTOMATIC. STOP PUTTING E11.9 ON FUCKING RX'S MEANT TO BE SENT TO THE BRIDGE PROGRAM. ITS E66 (FAT DUE TO BEING A FATTY) THERE ARE 3 DRUGS THAT ARE COVERED, LEARN THEM, AND STOP SENDING ME OZEMPIC WITH THAT FUCKING NOTE ON IT. YOU'RE DOCTORS NOT DENTISTS. This program isn't fucking hard, but there are hoops to jump through. I need a fucking drink.

If you are anywhere near a farm or livestock and you set off fireworks, you’re an asshole. I have horses running for their lives, calves circling and dogs going nuts.

Darryn Peterson with exactly the Summer League debut you think he’d have… Just an unbelievably gifted offensive player. It’s effortless. The most obvious day 1 star in this years draft, the Utah Jazz absolutely scored having him fall to #2. He’s a freak.

@mbelanger227 @Badhombre Well, a lot of it depends on how big the house is, how many people live there, etc. But sounds like you have a good deal going in Texas

@SteveP733300 @Badhombre Damn...you got hosed. Just paid my Texas bill for June electric and it was $65.87. And my A/C will be on and off during the day even when I'm at work because my cat likes to be comfortable as well...

@karenvanhoek @Badhombre @grok please answer this question. How does the electric company know how to break down the bill by different appliances and usages?

@karenvanhoek @SteveP733300 @Badhombre I'd like to know how the electricity for his laundry was more expensive than the charge for his refrigeration, which is ALWAYS on, AND in a separate category. His math isn't adding up.

I have little doubt the new Tesla Model Y L will quickly become Tesla's top seller. However, some people prefer more of a traditional SUV look. If @Tesla took the Model Y L and squared off the rear quarter, not only would it change the look, it would add more third row headroom and storage space. What do you think? @SawyerMerritt

@SteveP733300 @shutupomgplz @kevinnbass Getting an M.D./Ph.D. means free tuition for both med and grad school, and strong choices post-degree. There'd be more people applying for those programs if they were 'easy'. Anybody describing a former M.D./Ph.D. student as "rejected from med school" is prima facie idiotic.

A young white man was assaulted. UK police respond by assaulting him again, arresting him, then posting on social media that they did a great job. This seems to keep happening. The job of UK police seems to be to help criminals commit crimes. That’s crazy.

Let’s keep it real here I‘m loving my time in the US. I love the people here. I love the food here. BUT I‘m currently hearing Americans say they have better pizza than Italy and there I just have to stop you guys because that is nothing else than blasphemy 😭 You don‘t have to be the best at everything, it‘s fine to admit when others do certain stuff better. And nobody beats Italy at pizza. Not even close 🇮🇹🍕

@ErichMSchwarz @shutupomgplz @kevinnbass Disagree that most medical students couldn’t get into these programs if they tried. Many medical students could get in if they tried. The fact is most med students have little interest in the PhD.

@shutupomgplz @kevinnbass Getting into an M.D./Ph.D. program, as @kevinnbass did, is the opposite of "rejected from med school". M.D./Ph.D. programs are very competitive; most people accepted into med school wouldn't get into them if they tried. With that explained, care to reexamine your assumptions?

@DrDiGiorgio What’s your take on 36 hours straight? (regular workday, On Call for the night and then working regular day the next day.)

Working for 24h straight is actually safer than the alternatives. Emergencies don’t stop happening at night and handoffs between teams increase errors. Having continuity overnight from daytime team reduces those errors.
St. Wilding Gyres@wilding_gyres

It should be illegal for medical professionals to work for 24 hours straight. What an absurd practice. We don’t let pilots fly planes for 24 hours straight. Why do we let people who make life and death decisions for total strangers do this?
