I have a patient with Waldenstrom’s who has AEs on 2 BTKis, now on Benda. IgM down 50%, but stuck around 4500 after 4 cycles. Keep going? Something else! #lymsm

This! LMWH often still given at 6pm which means anti-Xa at midnight. Not good for anyone. No need to routinely monitor levels. Only very specific situations e.g. extremes weight, thrombosis despite normal dose, bleeding, renal failure #TeamHaem

I have recently attended several conferences Surprised to find many are measuring anti-Xa levels routinely in pts receiving LMWH. Why do this? - forLMWHs have predictable pharmacokinetics - the trials of LMWH did not measure levels so we don’t know the levels to attain ?
