He probably had some flesh removed from his body, likely his back or something to replace the wound and hide the scar.
Also, you'd be surprised at how fast the human body can heal, as someone who's been in surgery twice before, it can heal quickly.

I mentioned he also has access to the best doctors because he's rich. It'd make sense that he probably had some flesh removed from his body to replace his ear and cover the scar. People do that, especially with 3rd degree burns.

Not only would he have access to the best doctors in the world, his ear would heal quicker than if there was a hole directly through it.
