All the indications are that Oxford maternity services are horrific, and we already know that many babies have been killed by basic failings in care.
There are some features at OUH that are different from other maternity 'scandals' though. There must be further investigation.

Serious question:
If a system focuses on ‘learning rather than blame’, what mechanism ensures accountability when harm is avoidable and repeated?
Because multiple maternity inquiries suggest learning alone hasn’t been enough.

Idris developed jaundice shortly after birth. Despite reassurances, delays in care led to kernicterus brain damage and cerebral palsy.

As mother to a severely harmed child, this deeply concerns me. Sure, it would be helpful not to have to battle for years for answers as to what went wrong and why my child was harmed, but it has long since become clear that the only way to get answers is through the courts...

@JamesTitcombe Working together is often promised, but rarely delivered in full particularly in how family voices are used within inquiries themselves.
Families are often central in providing evidence, but far less involved in shaping...1/2
