Great question. Their mind doesn’t process it that consciously. They have a psychological defense mechanism that defends their ego from facing any reality it can’t handle. So when they see someone who has what they don’t or what they are lacking like a genuine sense of self or lack of fear of expressing oneself openly, moral integrity, etc, it doesn’t register in their consciousness as “they have better qualities than me”, their unconscious recognizes that person‘s differences could threaten to alert the conscious self that it is weak or faulty or less-than, so it simply sends a threat signal to the conscious mind without any reasoning behind it. So the narcissist only feels: THREAT/ANNOYANCE/DISLIKE. And they react to that feeling without ever questioning where it really comes from. They can’t. Because at some point way back in the pathological construction of themselves, they “told” themselves unconsciously never to question where their feelings originate because if they did they might discover that those feelings are actually self-deluding, and obviously they don’t want to know that. So their entire minds are completely structured around keeping themselves in a delusion of themselves and the world around them.
