@trevorclower Totally agree, as well as ensuring carers are supported whilst the person they care for is looked after as well. Their expertise is often overlooked, then there is the expectation they will take over after discharge without a second thought

PCN’s should use the Unpaid Carers experience to help them design a good care plan & the Unpaid Carer, fully informed, will ensure the treatment continues what the PCN has to leave the Patient. #WeNurses

There is a lots of data collected at our trust, time and energy is spent sending it to staff but there is no assurance process that it is being listened to or acted on. #WeNurses
