"𝘞𝘦 𝘢𝘳𝘦 𝘢𝘭𝘭 𝘵𝘰𝘶𝘤𝘩𝘪𝘯𝘨 𝘵𝘩𝘦 𝘴𝘢𝘮𝘦 𝘱𝘢𝘳𝘵𝘪𝘤𝘪𝘱𝘢𝘯𝘵𝘴." We are honoring #NationalMilitaryAppreciationMonth by highlighting Sheldon Andrews, Veteran Special Projects Manager at the Technical College System of Georgia (@TCSG) and @UniteUsHQ partner. Sheldon works with the Jobs for Veterans State Grant (#JVSG) program to provide priority services to #Veterans with significant barriers to employment. He emphasizes, realizing the true "no wrong door" concept means collaborating across agencies because we are all serving the same community members. We are thrilled to welcome Sheldon back to #OneContinuumGeorgia2026 next week as a speaker, where he will share more insights on how cross-agency workflows are key for connecting Veterans to employment and workforce opportunities. Stay tuned for more insights live on May 6th! 🚀🍑

“We're government so everybody wants a report about this or about that." But one shared record changes everything. No more retelling client stories. No more siloed data and case notes. Care navigators can see the full picture, specialize in what they do best, and confidently refer out to trusted partners. All while pulling real-time data to report on impact. Hear Christa Hernandez of Santa Fe City and Coy Maienza from Santa Fe County discuss what this looks like in action through the CONNECT program.

@UniteUsHQ and @Ready_Computing have announced a strategic partnership to strengthen care coordination and data exchange across New York's health and community care system. Two industry leaders, complementary strengths, one shared goal: building the infrastructure that communities, providers, and states need to work as one. Separately, we're each best in class at what we do. Together, we can do more than either of us could alone. This is what the future of health and community care looks like. Not single vendors working in isolation, but the right organizations coming together around shared standards, combining their strengths, and building systems that work across the full continuum of care. "We knew exactly who we were and exactly what we were providing." Our CEO and Co-Founder Taylor Justice explains to @KaganoffSari of Aytza why a single image of a fork in the road helped protect the Unite Us mission. We weren’t just building another resource directory. We were laying the foundation for a new kind of infrastructure: one that 𝘵𝘳𝘶𝘭𝘺 closes the loop, tracks outcomes, proves impact, and helps providers go the “last mile” for those they serve. It wasn’t just a metaphor. It was a call for our partners to move beyond convenience and invest in accountability, outcomes, and real system change. 🎧Hear the full episode: Apple Podcasts - apple.co/3OaHHKR Spotify - bit.ly/3QhHW7p Amazon Music - amzn.to/4tRMNuv #CEOPajamaTime #HealthTech #FounderStories

Did you know that April is #StressAwarenessMonth? But for those delivering healthcare and community-based care, stress isn’t seasonal. It’s a daily reality. Reducing #burnout takes more than recognition and conversation. It requires better systems. Because when we better support the people delivering care, we improve #outcomes for entire communities. → Want to see it in action? 📍Learn how we reduced stress for 71% of caseworkers and freed up nearly 600 hours in Summit County, Ohio: bit.ly/4cHoN6v

Last week, 47% of you voted to hear more about #workforce and access challenges in #ruralhealth. Yesterday on our first 𝐔𝐧𝐢𝐭𝐞 𝐔𝐬 𝐋𝐢𝐯𝐞, Dr. Lisa Patton and Katie Keating tackled those concerns head-on, discussing how to build a stable workforce while preparing for the looming CMS (@CMSGov) audits this August. 𝐊𝐞𝐲 𝐭𝐚𝐤𝐞𝐚𝐰𝐚𝐲𝐬: 💡 Rural health is facing an institutional crisis as much of the current workforce nears retirement. We must build "generational career paths" and tap into allied health roles to keep care sustainable. 💡With CMS audits of states’ #RuralHealthTransformation Programs just four months away, reporting "activity" isn't enough. Data infrastructure must be ready to prove real-world #outcomes. 𝐏𝐫𝐨𝐯𝐞𝐧 𝐑𝐞𝐬𝐮𝐥𝐭𝐬: Rural health transformation is possible. We’re proud to partner with Missouri #ToRCH program, which has already seen a 19.6% improvement in controlled blood pressure and an 18% increase in connections to behavioral health services by connecting clinical care with community services. Calaveras Health & Human Services Agency’s (@Cal_HHS) Justin Barnes, MPH hits on a fundamental truth: you cannot manage what you do not measure. 📈 Effective #ruralhealth transformation requires a shift from identifying community struggles to operationalizing the data behind them. Without the right infrastructure, lived experiences remain siloed, rather than serving as the strategic evidence needed to drive resource allocation and cross-sector #alignment. This isn't just about reporting; it’s about using data to better serve and support our communities. Catch the full webinar here: 🔗 bit.ly/3OpeKuE 🛠 Rural Health Solutions & Tools: bit.ly/43noQ3m

Health is about more than just what happens within clinical walls. It’s about the ability to live with dignity in your own home. 🏠 When 19-year-old Adam Larsson returned home after a life-changing accident, his family faced a new reality: navigating a world that wasn't built for a wheelchair. Through Missouri’s Transformation of Rural Community Health (#ToRCH) program, Bothwell Regional Health Center was able to "wrap their arms around" the family, connecting them with Solid Value Contracting to build a ramp and deck that turned a barrier into a bridge. Powered by Unite Us, the ToRCH model leverages the unique strengths of #rural Missouri to help neighbors like Adam get back on their feet. ➡️ Missouri’s ToRCH model is proving what’s possible when technology and community-led hubs unite: bit.ly/4thkhTh 🔗 Read Adam’s journey in the @SedaliaDemocrat : bit.ly/486AaDn
