Has anyone ever seen mtx level interference using homologous enzyme immunoassay while patient is receiving high-dose leucovorin (100mg/m2 q3h) ? #rxtwitter

@BijoyTelivala @drkeithsiau I don't believe there is any clinical differences when equipotent doses are used. Really comes down to cost but obviously patient specific factors (DDI and formulation for administration) are important.

@saraann9 Pom is a great option if there has been some time since last exposure. I'm torn on GPRC5D BiTe right after BCMA BiTE just in case T-cell exhaustion is the reason for minimal response with Tec

@VHarrisPharmD We utilize pomalidomide in these settings in hopes of a T-cell boost, however switch to a trial or GPRC5D product would be great options as well.

73 year old male with IgG kappa MM with normal cytogenetics, s/p 4 LOT including ASCT. Most recent release was biochemical only. Currently on month 3 of teclistamab with minimal change to m-spike and light chains. No CRAB Sx.

Because patient is asymptomatic and tolerating treatment despite minimal changes in myeloma serology, makes me a little more comfortable to hold for potential late response.

Final results
In MajesTEC-1 the median time to first response was 1.2 months with a range of 0.2- 5.5 months. With a population of 165 can't say for sure the distribution of patients who lean more towards having a later response.
