@Xeon4f145d96s1 As an FY1 on T&O I got fast bleeped by an IMG SHO who was a senior reg in his own country to manage his patient triggering sepsis because he didn't know what to do. Apparently where he qualified he could just right medical review in the notes and someone else would sort it out.

Given sometimes the FY1 still has to hand-hold the IMG JCF I really don’t see an issue.

With 5yrs of NHS experience for IMGs . An FY1 while you are a JCF / reg might end up being your Clinical supervisor when you eventually get into GP training.

Did you know that Advanced Clinical Practitioners (Podiatrists, Physios, etc..) are leading cardiac arrests in Plymouth hospitals, putting patient safety at risk? 💀☠️
⭕️ The Medical Registrar should be the only one leading the code, as patients might need time-critical treatments such as thrombolysis.
⭕️ This is outside the scope of practice for podiatrists and other allied healthcare professionals and requires complex decision-making.
⭕️ Patients might need a bedside Echo to rule out cardiac tamponade, good luck explaining that during the code.
All of this has been allowed by the ladder-pulling EM consultants working in Plymouth hospitals.

@DrJPGannon @Dr_DeanS A 24 hour on call would be working 24 hours. Calling them on call is a joke, as with the exception of my ICU job, every job I've had resulted in me working constantly if on medicine or getting bleeped at least hourly on surgery. On call implies you have rest, that's not the case.

Why do you need ‘accommodation after a shift’? Do the nursing staff request ‘accommodation’ after a 12-h night shift🧐
With an hours commute each way you’d be better off working a 24-h ‘on-call’ rota: less commuting time, better training opportunities, better continuity of care and much better WLB🧐

@Jirah_House Lying vs. sitting is an interesting one. It's an easier procedure sitting up, but for some reason us less-experienced ward staff seem much more likely to attempt it lying down. Probably a training issue.

102 - the number of hours left until voting for the next president closes (12 noon on Monday 14th April).
79 - the number of votes by which the 2022 presidential election was won.
1 - the number of candidates whose surnames allow for a witty (if slightly corny) campaign slogan.

@AuldM @ThereseHirst @DPJHodges @MFoxxer I haven't read the book so can't comment on it. But I'm sure the natural resources in Crimea had nothing to do with the land taken in 2014. As for Ukraine Nazis killing russian speakers almost half of Ukraine spoke Russian prior to 2022, so that sounds like Russian propaganda.

@DPJHodges @MFoxxer Yes, fair. Ukraine had the largest military in Europe outside of Russia. It was backed by NATO, traing, bombs, weapons and £billions. Russia has Every Right to protect Russian speaking people they liberated from the Nazis

If PA's and AA's suddenly didn't exist tomorrow all that would happen is you'd suddenly need more doctors because they're just being used in place of doctors.

I welcome the review into PA/AA roles announced by @wesstreeting. They are important members of NHS staff.
While the NHS is clear these roles are not replacements for doctors, it’s vital we listen to concerns, tackle them & find a way forward that’s right for patients & staff.
