@AirborneRxDr @BillAckman If the controller was working them on separate freqs, they might not have even heard each other’s advisories. Even less situational awareness.

As a former Blackhawk instructor pilot and current airline pilot, here is the breakdown: They never saw each other. The CRJ is just rolling out of a left turn to a very short final and both pilots are focused on the runway. Both pilots hear the Blackhawk being queried about seeing them and will maintain visual separation, not realizing the Blackhawk pilots are looking at the wrong jet (more on this later) The Blackhawk does appear on their TCAS however all TCAS breakout commands are inhibited on the CRJ below 1000 feet. Also, the Blackhawk has its landing light slewed straight down instead of outwards as required making it even harder to see. The Blackhawk crew was cleared onto a published route and from this video shows they were level and not climbing into the CRJ’s path (also more on this later). NVGs have 2 features within them called automatic brightness control and bright source protection. The ABC feature acts like a camera and dims the image as in this case where there are immense city lights. The BSP feature does the same except that it does this for singular light sources ie. the CRJs landing lights. Couple this with the intersecting flight path and now the CRJ appears as stationary lights that blend in with the city lights. As to being cleared onto the route, this never should have happened with traffic landing on runway 33. It is impossible to maintain the required separation between aircraft. With that said, the tower controller saw both of them and allowed this to unfold. A visual separation authorization does not alleviate the controller of averting obvious disaster. It was obvious to him what was unfolding and instead of acting, he stood by and watched as 67 people needlessly lost their lives.

Here's the telemetry for the IFT7 Starship failure. Looks like they lost one of the Raptors first at T+7:38, followed by another and a Raptor vacuum. Full loss of telemetry at T+8:27 or so...
