Many of us @trapwire have spent decades conducting CT ops around the globe and can attest to the fact that the lone wolf/self-radicalized opponent is by far the most difficult to detect. But it's not impossible. In most cases there is leakage – details of a plan that are accidentally (or purposefully) disclosed by the perpetrators to those around them.
