Does bladder fill matter for intact prostate RT? Some institutions (Georgetown) sim and treat with bladder empty! So I suspect it is one of the things we care about for no reason in #radonc

I wish some super smart basic scientist could come up with a test to differentiate benign bounce (bb) vs recurrence. Although if you play the odds… chance of bb is 25% < 3y and chance of failure for int risk pt after #SBRT is 0%

@ParikhSimul @yuanjamesrao That is a line written in an article discussing the false claims act, perhaps modifier 25? Bottom line, the root issue is prior authorization and insurance trying to dictate how patients are treated, almost always incorrectly.

If you generated IMRT and 3D-CRT plans for insurance comparisons, and only approved for 3D-CRT is it illegal, fraudulent, unethical or incorrect to just do the IMRT treatment if you think it's better and tell your billers to charge 3D? Same for IGRT? What would you do? #radonc
