Pelvic hemorrhage is deadly🩸☠️! You MUST have a strategy for managing these patients. ➡️ Treat open book pelvic fxs w/ sheet or binder ASAP 🟢  An effective binder is placed over the greater trochanters (not the ASIS). This decreases pelvic volume, ⬆️ the likelihood of tamponade.  ➡️The preferred treatment for pelvic🩸is arterial angioembolization. ➡️ Pelvic packing should be used when endovascular treatment is not available, when rapid intervention is required & packing offers faster hemorrhage control than angioembolization, or when the patient is being taken to the OR emergently for other reasons. How to perform preperitoneal pelvic packing 👇🏼👇🏼👇🏼👇🏼 ➡️ 8-10 cm lower midline incision that extends to the superior aspect of the pubic symphysis. ➡️Incise the fascia while keeping the peritoneum intact ➡️Carefully pack the preperitoneal space (of Retzius) w/ lap pads, taking care to avoid injuring your fingers on bone fragments.  Typically, 6 lap pads are placed in an adult pelvis (3 on each side). ➡️Temporarily close the fascia or skin with running suture.
