The glans definitely isn’t necessary for orgasm in women or in men.
However, sufficient stimulation of the external clitoris or penis does seem to be necessary.
When the glans is amputated or the innervation is cut, direct external stimulation becomes more necessary.
The situation is far more studied in men, though far more women have various levels of amputation.
In men, the way it works is the more of the external penis is removed, the less likely orgasm is. I’m open to being challenged on this, but I think it’s pretty well established.
I think it works the same for women, and this is why it’s a big problem that FGM survivors aren’t categorized based on level of damage to the actual clitoris.
Among the women who have contacted me over the years, of all the who women lost external sensation, no one has been able to compensate by orgasming from internal stimulation. Those who had penetration orgasms before lost them.
What’s really key is that penetration becomes less pleasurable with external damage to the labia minora (if over excised), frenulum, and/or clitoral body and its nerve supply to the glans.
