Contrast-associated AKI matters after EVT. In the CAN-REST collaboration (>6,600 patients), ~5% develop CA-AKI with nearly 50% mortality.
The new CAN-REST score helps early risk identification.

Glad that @PittStroke team contributed to this @GreenJournal study across 73 centers in 16 countries to assess and predict the risk of contrast-associated acute kidney injury in patients undergoing EVT.
👀 Risk was 1/20 and a risk score was validated.

MAPSTROKE analysis in Italy highlights how data-driven monitoring can improve #stroke care coverage. Progress in #reperfusion therapy is evident but Stroke Unit access, which should be guaranteed to all stroke patients, remains the key gap.

@ZiniAndrea Both medium and distal occlusions should be randomized, and selective enrolment avoided. I agree DMVO classification isn’t always straightforward. Anyway, even in LVO, EVT for very low NIHSS remains debated. Let’s study it. 😃

@a_ciacciarelli As can be clearly seen from the CTA (and baseline DSA that I attached) the occlusion is not distal. A correct understanding of the vascular anatomy and the related parenchymal territory is crucial to avoid placing the patient in the wrong category.

@ZiniAndrea Especially regarding EVT, I would be cautious in distal occlusions and low NIHSS, given that evidence of efficacy is still missing, and there is probably more to lose than to gain, given current device technology.
