A 30-year-old patient presented 2 days after blunt left eye trauma with traumatic mydriasis and a 1-mm hyphema. Funduscopy (A) showed temporal peripheral retinal concussion with blot hemorrhages. High-resolution foveal OCT (B) demonstrated the angular sign of Henle fiber layer hyperreflectivity, associated with ellipsoid/interdigitation zone focal disruptions, together with a thin central vertical hyperreflective line at the fovea. These findings are most consistent with acute globe deformation after ocular contusion, causing focal foveal stress and transient changes in the Henle fiber layer. A, B, Ophthalmoscopy and infrared autofluorescence revealed a retinal hole nasal to the optic disc (arrowheads) with an adjacent oval, translucent, nonpigmented cyst (white arrows) located anteriorly in the posterior vitreous cavity of the right eye. C, OCT demonstrated a full-thickness retinal hole measuring 1041 μm and a floating vitreous cyst with an isoreflective wall and hyporeflective lumen. Laser photocoagulation was applied to the margins of the retinal hole, and close follow-up was maintained. A 14-month-old boy was referred after routine ophthalmic screening detected abnormal fundus findings. Over several months, progressive developmental delay and hypotonia were noted by his parents. Widefield fundus photography demonstrated bilateral macular cherry-red spots with surrounding retinal pallor (A). Fluorescein angiography showed mild relative foveal hyperfluorescence without leakage or peripheral vascular abnormalities (B). OCT showed mild thickening of the inner retinal layers in both eyes (right eye [OD], C; left eye [OS], D). Genetic testing identified pathogenic variants in the HEXB gene, confirming the diagnosis of infantile Sandhoff disease. To overcome iris occlusion, this study used a transscleral approach for AS-OCT imaging. Despite scleral density, clear images were obtained because of low pigment content and minimal light absorption.
