Yet again we're told 'we already know what the problems are' in maternity, as a reason against a public inquiry. It's almost ten years since the Morecambe Bay report was published and it's clear that 'knowing what the problems are' hasn't made any difference nationally, and at @UHMBT the main effect may have been to make them better at covering up, as shown by Ida Lock's death and her parents' fight for truth. Because in maternity care it's not as simple as knowing then learning and changing - there are bigger, more complex issues at play. There are people who don't want to change. There are failed regulators who make deadly failure consequence free. There are NHS leaders more concerned with reputation than lives. And so many more barriers to meaningful change which we do not fully understand, and do not fully understand how they all interact/feed each other and end in avoidable death and harm. But given his previous role at HSIB I can understand why Jimmy Walker doesn't want a public inquiry - his decisions and actions would be under the microscope too, and the findings may not be particularly flattering. bbc.co.uk/news/articles/…
