Annoying OR moments: WHY OH WHY is it NEARLY IMPOSSIBLE to get quick & rapid access to this OP (oropharyngeal) airway that is likely to be needed in an emergency or an “I need it now” airway situation? Surely this packaging can be done better! #anaesthesia

@DrShmuk @amit_pawa Gosh. 6l/min is high and seems like a waste of oxygen. May as well use an icu ventilator without soda lime. Saving soda lime for the planet sounds worthwhile. But not if it destroys the planet because of the FGF used. Also the patient - humidity and temperature of airways.

TIPS for TIVA: Planet edition 🌎🌍 Based on clinical studies, we should be looking at 6 litres/mn total flow to hit the sweet spot between soda lime usage/canister longevity, oxygen use and production costs and global warming potential of all of the above. Is it your routine?

(1) similar trends as benchtop study (2) high FGF = significant cost saving (3) for non laparoscopic surgeries - lower FGF (1 or 2) probably better for 🌏 (4) for laparoscopic sx - higher FGF MIGHT be better for 🌏 (5) high FiO2 negates environment benefits of high FGF. 8/

@Mon4Kooyong I would love to be able to get a booster vaccination for my children aged 6 to 15. But it’s seemingly impossible at the moment. Can you please lobby ATAGI to update their recommendations? Boosters are being given in other countries around the world.

All patients fine and got for home today. Reflecting: this helps hugely with two of the very few things #nhs clinicians can control EFFICIENCY & MORALE. On that basis it’s worth it. We learn how to work in parallel and everyone felt part of something special.

@chrisgj198 @CO2Guerrillas @YouAreLobbyLud @ACTEducation I have a CO2 monitor in my classroom along with 6 ceiling fans, 2 standing fans, all windows open, and 2 Corsi-Rosenthal boxes on high and am able to keep my C02 levels below 500ppm. Not allowing teachers to protect themselves and their students is criminally negligent at best

@GasNovice I think very little known about normal BP in well (young) children. Cuff BP is often unreliable, as we all know, and invasive pressure is just not an option for research in healthy kids. Be interested to hear what others know.

#paeds question… Is there a target BP for neonates and infants intra-op? I’ve read about target sBP of >55 from a study on sevo vs spinal. I’ve also heard a rule of thumb being target MAP = post conceptual age Any evidence out there?
