Obesity is masking heart failure — and we’re missing it ⚠️ #HF2026
PORTHOS study: largest population-based HF prevalence study in Southern Europe (n=6,189, 1 in 6 adults ≥50 has HF)
Key finding: BMI shows a continuous inverse association with NT-proBNP — ~50 pg/mL less per 5 kg/m² increase
Obese patients with LOW NT-proBNP (<125) vs lean patients above threshold:
🔴 More HF symptoms: OR 1.97 (p=0.013)
🔴 LA enlargement: OR 3.50 (p=0.010)
🔴 Echo abnormalities: OR 3.63 (p=0.016)
Obese patients below the NP threshold carry GREATER HF burden than lean patients above it

Therapies including:
• GLP-1 receptor agonists
• SGLT2 inhibitors

Fascinating slide from #HeartFailure26 🫀 People with CVD, hypertension & diabetes are at high risk of developing heart failure — and the comorbidity burden is staggering:
• ~63% of HF patients have hypertension
• ~56% have CAD or ischemic heart disease
• ~50% have CKD
• ~48% have hyperlipidemia
