The ACC/AHA has released new guidelines for the management of dyslipidemia; the previous edition was published in 2018. The new guidelines for primary prevention base decisions on the PREVENT score, where moderate risk is defined as 5–10% and high risk as greater than 10%. They also recommend testing all adults for Lp(a) at least once in their lifetime. In addition, there are some changes regarding LDL-C goals. For the full guidelines: ahajournals.org/doi/epub/10.11…

SURPASS-CVOT unpacked ⬇️ In >13,000 adults with long-standing T2D + established ASCVD followed for a median 5.3 yrs, once-weekly tirzepatide went head-to-head with dulaglutide in the largest and longest incretin CVOT to date. Tirzepatide reduced 3-point MACE (CV death, MI, stroke) by 8% vs dulaglutide (12.2% vs 13.1%; HR 0.92; 95.3% CI 0.83–1.01), meeting the primary objective of noninferiority for CV safety on top of guideline-directed therapy. Beyond MACE, tirzepatide delivered a 16% lower all-cause mortality (HR 0.84; 95% CI 0.75–0.94) and fewer heart failure events, suggesting broader cardiometabolic protection in this very high-risk population. Metabolic efficacy clearly favored tirzepatide: mean HbA1c reduction ~ -1.7% vs -0.9%, and body-weight change ~ -11.6% vs -4.5% compared with dulaglutide, with greater improvements in triglycerides and other risk markers. #SURPASSCVOT nejm.org/doi/10.1056/NE…

في مؤتمر جميعة القلب الآمريكية 2025 عُرضت نتائج دراسة لعلاج enlicitide كأول علاج عن طريق الفم لمجموعة PCSK9 التي تستخدم لعلاج الكوليسترول. العلاج أظهر انخفاض في مستوى الكوليسترول الضار LDL تُشبه تلك التي تُحققها الأدوية القابلة للحقن من نفس المجموعة (أليروكوماب، إيفولوكوماب و إنكليسيران) للمشاركين في الداراسة الذين يعانون من جلطات قلبية أو دماغية سابقة أو لديهم خطر إصابة عالي. أيضاً النتائج أظهرت إنخفاضاً بمستويات الكوليسترول الاخرى الهامة مثل Non-HDL, ApoB وانخفاض بسيط بمستوى Lp(a). وجود علاج عن طريق الفم لهذه المجموعة يعطي خيار جيد لمن يرفض الحقن.

Is statin safe in pregnancy? Animal studies have indicated potential teratogenic effects of statins; however, their impact on human fetuses remains unclear. Evidence from a Norwegian registry involving over 800,000 pregnant women found no significant link between first-trimester statin exposure and congenital malformations . Therefore, patients who conceive while taking statins may be reassured that the risk of birth defects appears not to be elevated. The OASIS-4 study has shown the result of a 71-week RCT of oral semaglutide 25 mg daily in adults with overweight/obesity without diabetes, showing clinically meaningful weight loss by week 64. Mean weight reduction reached approximately −13.6% at week 64 with oral semaglutide 25 mg, placing it in the double‑digit range for oral therapy and advancing non‑injectable options for obesity care. Oral GLP‑1 options have the potential to broaden access and improve adherence for individuals who favor oral medications. nejm.org/doi/full/10.10…

ESC/EAS has updated the clinical guidelines for the management of dyslipidemia. Risk categories have been changed based on SCORE2 risk ( moderate risk up to 10% and very high risk has been categorized for >20% of SCORE2). Also recommend adding bempedoic acid to the maximally tolerated dose of statin, or even in patients who are unable to take statin therapy, to achieve the LDL-C goal. academic.oup.com/eurheartj/adva…

Today, the Canadian Adult Obesity Clinical Guidelines have updated their recommendations for pharmacological management. They now emphasize the use of measures of central adiposity, such as waist circumference, waist-to-hip ratio, and/or waist-to-height ratio, alongside ethnicity-specific BMI thresholds and/or adiposity-related complications, to guide decisions regarding the initiation of pharmacotherapy. Setmelanotide has been included in the guidelines for managing obesity related to genetic conditions. Additionally, new recommendations have been released for individuals with obesity who also have MASLD and MASH, sleep apnea, and knee osteoarthritis, specifically for those with a BMI greater than 25. ثلاث دراسات حديثة نُشرت لنوع آخر من الانسولين الاسبوعي المسمى إفسيتورا (efsitora) ومقارنة نتائجه بالانسولين اليومي طويل المفعول على مرضى السكري النوع الثاني. نتائج الدراسات أوجدت: - مستوى السكر التراكمي متقارب للمجموعتين بعد 52 أسبوع (QWINT-1) - حالات إنخفاض معدل السكر أقل مع إفسيتورا - إحتياج تعديل الجرعات للوصول للهدف كان أقل مع الانسولين الاسبوعي وأيضا مجموع الجرعات الأسبوعية كان أقل مع مجموعة إفسيتورا - متوسط جرعات الانسولين قصير المفعول واحتياجها كانت أقل مع الانسولين الأسبوعي مقارنة بالانسولين اليومي (QWINT-4) nejm.org/doi/full/10.10…… thelancet.com/journals/lance…… thelancet.com/journals/lance…
