Monitoring Hypervolemia and Guiding Deresuscitation 🔹 Clinical Signs • Elevated blood pressure (mean, systolic, diastolic, pulse pressure) • Decreased heart rate, increased jugular venous pressure • Peripheral edema, anasarca, altered mental status • Positive fluid balance, orthopnea, and hepatojugular reflux • Daily weighing and urine output trends (incl. furosemide stress test) • Pulmonary rales (crackles) are non-specific; consider multiple signs in tandem 🔹 Fluid Stewardship Approach • 4 D’s: Drug, Dose, Duration, De-escalation • 4 Questions: When to start/stop fluid therapy, and when to start/stop fluid removal • 4 Indications: Resuscitation, Maintenance, Replacement, Nutrition • ROSE Model: Resuscitation, Optimization, Stabilization, Evacuation ⚖️ These principles help guide safer fluid management and timely deresuscitation, akin to antibiotic stewardship.
