Here it is: My official guide to the difference between clinical notes and the After Visit Summary.
- When clinical notes are shared w/patients it is called #opennotes. Lots of evidence that notes are helpful for patients and care partners.
- The After Visit Summary is a joke.

UPDATE: 12 people are dead after a mass shooting inside a bar in Thousand Oaks #California. One of the victims is a sheriff's sergeant who went in to help. The suspected gunman is also dead. Dozen others injured. @WTNH

evergreen reminder – when someone stops seeking a cancer cure, they are not giving up all “treatment” or “medical care” – palliative care, hospice, pain & symptom management are care – they aren’t curative, but they are still important treatment. #hpm #palliative #cancer
