MGUS and asymptomatic Creatine Kinase elevation. After ruling out statin use or hypothyroidism, is there any association between plasma cell disorders and CK? Macro CK type I is CK associated with immunoglobulins. Very rare Nemaline Myopathy. Finally AL Amyloid of muscles.

A post caught my eye so commenting here. Antibiotics derived from Streptomyces are called “Mycins” with a Y. Like Streptomycin. Antibiotics from micromonospora end with “micin” with an i like in Gentamicin. So there is no Y in Gentamicin.

@aditya_gan3500 Long time ago I learnt that the orbitalis muscles guarding superior and inferior orbital fissures are innervated by sympathetics and act as valves for retro orbital venous plexus drainage. In flaccid paralysis of orbitalis muscles retro orbital venous plexus collapses. 🤷🏽‍♂️I am old

Not uncommonly SPEP is checked for neuropathy and a diagnosis is made with MGUS. Is it related? If it’s Axonal then check for cryoglobulin. If demyelinating then usually IgM or with Anti MAG. POEMS is clinical and usually Lambda. Don’t forget Amyloid. Axonal IGG Kappa Not related

A patient with HIV with #PJP pneumonia on inhalation Pentamidine presents with spontaneous pneumothorax and Xray shows upper lobar #pneumatocoeles and #lateralpneumonia. What happened? Due to aerodynamics, upper lobar distribution of Pentamidine is inadequate causing above change

@Sage_medics Find the dimple in a prone position. Go a little superior and lateral from the dimple and voila! The best place for bone marrow aspirate and biopsy on the superior iliac crest.

@DrAkhilX Pentamidine for PJP is OK after transplant/CAR T. Switch to Bactrim when counts OK. Atovaquone is OK except taste. Dapsone is good but monitor for Methemoglobinemia. Long term inhalation especially in HIV patients can cause lateral pneumonia. Can cause upper lobe pneumatocoeles.

@RenoHemonc The purpose of my message was that to document CR/MRD, one would need a bone marrow biopsy. With Blood tests and urine, even if all negative one can tell only VGPR and not beyond. With immunotherapy, persistent MRD neg status at less than 10 to the power 6 is very comforting.

In Myeloma, if the M protein at diagnosis was 4g/dl; Very Good Partial Remission or VGPR is defined by >90% reduction. Hence, just move the decimal to one place left. In this case M protein <0.4 g/dl would be VGPR. Even if SPEP/IFE negative, to document CR, we need Bone Marrow.
