@Ricky_Turgeon @DrToddLee @BlairMacPharmD @medmyths Oh I am! But I dont think the trial will be long enough or high risk enough. I just think HF is a very low risk, CKD is a very low risk, even CAD is a very low risk in "new dx type 2 dm". But retinopathy. Just need 1% a1c difference for the curves to separate.

There is at least one trial (SMARTEST) looking at this question, but it is too short I think to show any meaningful difference. I like the Diabetes Canada approach.

@Ricky_Turgeon @DrToddLee @BlairMacPharmD @medmyths Metformin wins. In the primary prevention population, sglt2i will have only small absolute macrovascular benefit (if it can even be shown). The microvasc - glycemic link is strong and easier to show. And metformin is stronger than sglt2i at glucose lowering.

@DocMCohen @ShopprsDrugMart The cherry is when the patient leaves your clinic after a 30 minute comprehensive chronic care visit, fills their prescription, and you receive the medscheck fax the next day....essentially reminding you what you prescribed?

@DrJacobsRad @Royal_College @fordnation Is it though? In a world in which a computer can make better and faster medical expert decisions than I can, maybe we need to prioritize being a good person. Not to mention that medical expert expertise is easy (and easy to teach) Being good & just is hard (and hard to teach).

@supermarioelia I counted my faxes today. Average 8 per day for the past 3 weeks. Half had suggestions (prepopulated). Only one had a contextualized suggestion: "Liraglutide and dulaglutide temporarily covered by NIHB" Which is huge news for anyone who works in high first nations' populations.

Love the notes from pharmacy that "Ozempic is on back order", with no other patient-specific suggestions. Is Trulicity/Saxenda/Victoza available? Is the patient covered for those options? I cannot fathom the # of these faxes being sent, and the time being wasted by family MDs...

My primary care doc bills my insurance $100 when he does an EKG in-office. He wanted another, I said could I go to my local hospital to get one? "Yes." I went by, they said would do it. I asked how much-- $1,200! I said no thank you. My doc did it. Time for uniform pricing in US.

I am not convinced virtual walk in clinics are improving anything at all. "Please see for DM2. I checked the A1c. It is 10%. I have done nothing except write this referral to someone with a 2 year waitlist and refill metformin."
