Hospitals are under pressure with many patients requiring admission for further investigations/treatments. If a loved one is deemed medically fit for discharge please work with us to facilitate their discharge in a safe & timely manner to free up beds for those who need them most

Our multi disciplinary teams work collaboratively to ensure you have a safe and timely discharge from hospital back to your own home or other residential setting

We have 78 rehabilitation beds, 48 in Waterside Hospital in Derry and 30 in Omagh Hospital and Primary Care Complex These allow patients to leave Altnagelvin Area Hospital and the South West Acute Hospitals following their acute episode of illness to continue their rehab journey

📣 Hospital Discharge Public Appeal If you, or your loved one, are in hospital and are told by a doctor that you are medically optimised to be discharged, then we need you to work with us to get you home, or into an appropriate community care setting
