Early CKD in young adults with congenital solitary kidney is often missed. Standard tests (eGFR, proteinuria, BP) can look normal. This study shows how renal scintigraphy helps detect silent damage. Better follow-up starts with better tools. Read: doi.org/10.1093/ndt/gf…

Four more important conclusions are: 1. The traditional lesion patterns MCD/FSGS are clinically unuseful to inform about how to treat, 2. More antigens exist, 3. Autoimmune, genetic and adaptive are the main DD, 4. Autoimmune podocytopathies have a good prognosis.

Look how much more important is to know if a patient is genetic or has anti-slit antibodies than if has MCD or FSGS at kidney biopsy. We have finally come to the era of autoimmune, genetic and adaptive podocytopathies. No more MCD or FSGS used as diagnosis!

Our new study is out! By STED microscopy on kidney biopsy and ELISA assays we show 1/3 of pediatric and half of adult patients (more in primary FSGS) have autoantibodies against slit antigens different from nephrin. i.e. anti-slit Abs kidney-international.org/action/showPdf…
