@mcwimberly It’s a great question- wonder if our @CDCgov domestic malaria friends have thoughts on why we haven’t seen (curiously the last year of this graph happened to be the last local case reported until now)? Importation keeps increasing (cdc.gov/mmwr/volumes/7…) so it seems surprising

This is some pretty important contextual information w/ respect to the recent report of local #malaria transmission in the United States. Is there a reason why we haven't seen any for the past 20 years? Or have we just not been paying enough attention?

@Fredros_Inc @NoorAbdisalan I'd argue that the last 20 years with no local transmission is more anomalous than the recent outbreak! Scott Filler published this back in 2006 showing local cases by year... we even had a cluster here in NYC back in 1993: cdc.gov/mmwr/preview/m…

That's an interesting observation. Thank You Justin. ~ Goes to show how malaria surveillance continues to be important even long after elimination (The United States was declared malaria-free by WHO in 1970: who.int/teams/global-m…) Even more importantly, it show how expensive and how complicated it can be to tackle these sporadic malaria incidences. As Filler et al states: "Although these locally acquired mosquito-transmitted outbreaks frequently involve only a limited number of infected persons, they frequently raise concerns in the community and require substantial public health resources. For example, as a result of the most recent local outbreak of eight malaria cases in Florida in 2003, reverse 911 telephone calls (a community notification system) were made to approximately 300,000 residents; insect repellent, postcards, flyers, and posters in multiple languages were distributed; public announcements were made through the media and to schools and homeless shelters; and notifications were sent to local hospitals and physicians to inform residents of that community. cdc.gov/mmwr/preview/m…"

My friend @NoorAbdisalan reminded me yesterday that Florida's climate has always been suitable for tropical diseases including malaria. ~ This suggests the recent cases of malaria there might not necessarily be a result of climate change. Moreover, such occasional local infections are not necessarily "black swans" in such areas. They are just very rare, but should be expected given all the global travel and the obvious receptivity (presence of competent vector species) in the area

A great illustration, I think, of two critical concepts: 1) seeing mostly indigenous cases doesn’t negate the importance of importation for driving transmission; 2) efficient strategies will require coordination across connected catchments- going it alone won’t get the job done.
