『AIエージェントには身元がいるか』 Microsoftは社内で動くAIエージェント群を、人間の社員と同じように管理する方向性を打ち出しました。 エージェント1体ごとにIDを与え、権限を割り当て、何をしたかの記録を残すという考え方です。 この動きは、医療現場のガバナンス議論に決定的な問いを投げかけています。 病院の現場では、人間スタッフですら「誰が・いつ・どの権限で電子カルテを開いたか」のログ管理が曖昧な施設が少なくありません。 IDを貸し借りしていたり、複数のスタッフで同じアカウントを使っていたり、個人の識別と実行者が一致していないことが日常的に起きています。 そこへ現在、議事録の作成、問診の補助、退院サマリの下書きなど、AIエージェントを業務に入れ始めています。 エージェントは人間より速く、休まず、同時に何件ものカルテを開いて情報をまとめることができます。 しかし、誰がその操作をしたかの記録がなければ、問題が起きたときに原因を特定できません。 従来のAIツールは「医師Aがツールを使って文書を作成した」という形で、作業者の個人IDが監査証跡の一部になっていました。 ところが自律的に動くエージェントは、医師の指示を起点に、自分でデータアクセスの判断を下して操作を実行します。 エージェントごとに明確な身元と行動ログがなければ、誰がその操作をしたかを後から追えなくなります。 アクセスログだけでなく、「エージェントXが、いつ、どの患者のデータを参照し、どのような内容を生成したか」まで追跡可能にする必要があります。 これは個人情報保護法の観点からも不可欠です。 個人情報の取り扱いを「誰がしたか」特定できる体制が整っていない状態で、自律的なAIを業務に入れることは、リスクを拡大させます。 まず、人間のアクセス管理を整えることが、AIエージェントを安全に入れる前提になります。 誰がいつカルテを開いたかが後から追える状態を整えることで、エージェント管理の設計もその延長線上で考えることができます。 エージェントにはIDを与え、実行した操作すべてを記録する。 カルテの鍵を誰が開けたか、記録が残る仕組みをAIエージェントにも同じように適用する。 そこから始めることで、医療現場にAIエージェントを安全に迎え入れる道が開けます。 x.com/vc_corner/stat…

talked to 3 cardiologists since this and im left with the obvious question:
why isnt the entire population on statins?
LDL cholesterol is cumulative damage and irreversible
the lower it is the better (without threshold)
