@KailashKolhe Great questions. There is a hypercoagulable state caused by infection. Pet CT showed multiple small liver abscesses and also inflamed pancreas. Blood culture showed e coli. Improving with high antibiotics

28/F with 2.4 cm cyst in head of pancreas. EUS shows a nodule within cyst which takes up contrast. Biopsy shows abundant mucin and columnar epithelium. MPD is not dilated. Surgery or observe? #GITwitter

@docdhir @Mister_trip Enhancing nodules are concerning to me. Would certainly be something to see a surgeon about. Plus at the age of 28 they’ve got a looooong time to be surveyed. If they were 78 it might be a little different…

@_vbalaban Agree. The surgery would be Whipples in this case.

@docdhir The enhancing mural nodule is a significant high-risk feature, considering also the young age of the patient, surgery is advised by guidelines. But sometimes the surgeon is hard to convince in such cases.

@nawrashatif1 Looks extraduodenal cystic lesion. Biopsy and fluid aspiration should give diagnosis

Please Sir Your 1st impression regarding this incidentally discovered subepithelial mass in 3rd part of duodenum ( distal to the papilla ) . The patient is 70 years old and the mass was seen on abdominal CT for the evaluation of prior urological problem .

@nawrashatif1 Looks like a submucosal lesion. Will need EUS to further characterize it

I didn't recognise the layer of origin or whether the lesion is truly intramural or extrinsic compression ....@docdhir
