Last session of the term 1. if FIT <10mcg cancer risk 0.3% therefore below USC pathway threshold. 2. However remember if FIT neg but IDA / bleeding / worrying symptoms may still need referral 3. Primary Care IT safety netting might help and funded for all C&W practices.

@ObidinanwaD Sorry to hear this happened, I was always taught (20 years ago) clinician should be closer to door but I don't honestly know anywhere that happens. Think just easier to have patient open door and sit right down.

Incident at work yesterday prompting a review of consultation set up. A mental health patient threatened to kill the ENP during triage, and she had to run out of the room before the patient locked herself in. She was violently screaming in the room. Police had to be called to break in, and bring the patient out. In this scenario, the patient was closer to the exit door than the clinician, and she may have been locked in if she didn’t get to the door very quickly to escape. It may be safer for clinicians to always be closer to the exit than patient. Medics, what is your current consultation room set up? Thoughts on this

Then we went on to learn about Prevent, modern slavery and ICON a campaign to support parents and help stop baby injury. Thanks to the safeguarding team and tutors, still 460 on at 4.30pm and lots of questions, so shows the engagement was excellent.

3. Cluster headaches - 80% smoke - its a big risk factor so try and stop - also means can use oxygen if needed. My head is spinning with so much information to digest!

1. Risk of medication overuse headache (MOH) if using paracetamol or NSAID for >15d / month for >3m. Can even get with triptans and opiates too. So short sharp treatment best. 2. antiemetics / prokinetics might be needed for acute migraine even if not actually vomiting

Learning points from this weeks meeting on Pancreatic Cancer. 1. On diagnosis in England 7/10 people are for palliative treatment only 2. Common mis diagnoses IBS, gallbladder, heartburn, muscular so ask more about bowels and consider CT TAP
