Donanemab for Alzheimer’s disease (now launched in India) Donanemab is a newer anti-amyloid antibody designed to slow progression of Alzheimer’s disease by clearing amyloid plaques from the brain. ▶️Who may benefit? Not all dementia patients benefit. Donanemab is best suited for: ✅Early symptomatic Alzheimer’s disease ✅Mild cognitive impairment (MCI) due to Alzheimer’s ✅Mild Alzheimer’s dementia ✅Amyloid-positive patients (confirmed by Amyloid PET/CSF or blood biomarkers) ❌ Not meant for advanced/severe dementia. ▶️What are the expected benefits? Studies suggest that patients treated with this drug have slower cognitive and functional decline. Some patients may remain independent for longer. ▶️Important points to note: 🔸It is NOT a cure 🔸Does not reverse established dementia 🔸Benefit is modest, but clinically meaningful in selected patients ▶️Dose & interval 🔸Given as IV infusion every 4 weeks. 🔸It is usually continued until amyloid clearance targets are achieved (often ~12–18 months), guided by specialist evaluation and imaging. 🔴Major safety concern: ARIA The most important adverse effect is Amyloid-related imaging abnormalities (ARIA), which can cause brain swelling (ARIA-E) or small brain bleeds (ARIA-H). Symptoms may include headache, confusion, visual symptoms and seizures (rarely) Hence, MRI monitoring is essential during treatment. Risk may be higher in: 🔸APOE ε4 carriers 🔸Patients on blood thinners Cost in India Pricing in India is evolving, but treatment is expected to 11-12 lakhs of rupees annually, making affordability a major challenge for many families. Take Away: ✅Donanemab represents an important scientific advance because it targets underlying Alzheimer pathology rather than only symptoms. But: ✅Careful patient selection is critical ✅Expectations must remain realistic ✅Lifestyle measures, cognitive engagement, vascular risk control, sleep, hearing correction, and caregiver support remain extremely important.
