Why was whole blood so attractive? One bag, simpler logistics, faster administration, easier use when access is limited, and a cleaner route to balanced prehospital resuscitation. SWiFT tested whether those advantages translated into better patient outcomes.

Huge credit to the patients, families, air ambulance clinicians, transfusion laboratories, trauma centres, NHSBT, and the full SWiFT Trial Group. Large, pragmatic prehospital RCTs are difficult to deliver - and this one moves the field forward.

Secondary clinical outcomes were broadly similar, and no clear safety signal issue emerged for whole blood. Thrombotic events appeared similar between groups.
