I am living my research HARD, you all. In OR helping establish paid home care routine for my parents #dementia #MCI to meet their preferences for Dad to remain at home, touring memory care units for a back up plan AND completing a R & R on the relationship b.t. 'home time' & QoL.

Our broad recommendations: (1) formally recognize home care and home care workers as part of the larger healthcare system, (2) explicitly include home care in emergency planning, and (3) strengthen worker protections while improving agency capacity. More specifics in our article!

@NathanForCH Great question! What community-based services are you targeting (health coverage, nutrition, housing, psychosocial etc)? Wondering if studies on community-clinical linkages may have useful measures? #__ffn_sectitle" target="_blank" rel="nofollow noopener">ncbi.nlm.nih.gov/pmc/articles/P…

We also examine the limitations of so-called "value-based care" and what "quality" means to patients, families and payers - an important consideration as the home care sector moves toward quality-based payment models.

Thanks to collaborators Mei Fong, @djr5901 and @tedlavie for this analysis of challenges facing home care providers and driving inequities in home care. Key takeaway: interventions promoting equitable, high-quality care must be co-designed with communities, workers and patients.
David Russell@djr5901

My colleague Mei Fong and I recently published a paper in the Gerontologist with @ezosa & @tedlavie which examines contextual forces shaping access/use of home care over the past decade, including multiple sociopolitical and infrastructural factors doi.org/10.1093/geront…

Grateful to my fantastic collaborator and @bcnuej for helping us think about how private dollars are shaping neighborhood healthcare access, for who, and what that means for health equity.

When family caregiving and medical care were disrupted during #COVID19, #homehealthaides were still there, anchoring care teams. Aides were "absolutely critical", providers and agency leaders told us. "Because of them, people were able to stay at home."
JGIM Journal of General Internal Medicine@JournalGIM

Though often invisible to medical teams, #homehealth aides were indispensable boots on the ground during #COVID19, delivering hands on care, coordinating medical services, and offering emotional support to lonely and anxious patients. #primarycare @ezosa link.springer.com/article/10.100…

And importantly, organizational strategies to support workers can only go so far without adequate staff. Administrators and CNAs in our study both said staffing shortages were their biggest fear moving forward. Improving the quality of #CNA jobs and work environments is crucial!

We also found that efforts to support direct care workers were more often quick fixes than structural changes. As one administrator told us, "It's like being in a war zone, and you're on the frontline saying, 'well, here's a pizza'". Recognition only goes so far.
