54/m on MHD x jan 2025, via right IJV temporary cath Not made AVF yet came for TCC B/l IJV Doppler was normal & patient was taken up with the plan of Right IJV TCC & The wire was not crossing 🧐🙂‍↕️

Pt on MHD x 2 yrs, Left BC AVF, Now c/o difficult cannulations, low flows x 1 month, USG -multiple stenosis areas in Arm cephalic vein fistulogram cannulating distal end of vein Confirmed stenosis & did plasty Post plasty improved flows Able to cannulate & HD

Couldn't cannulate right IJV due to Mid ijv complete block , left IJV also stenosis So Put a 18G jelco in right EJV & contrast to know the down path before planning a TCC
