@AgarwalRajivMD I use patiromer and continue Finerenone. Strict K restriction is imperative, but sometimes difficult to pull off. Would be judicious in Stage 4 patients and counsel the patients. 3a is the best place to start; with RASI, SGL2, MRA combination

Use finerenone in ckd with t2d if k at most 5 and egfr >25. Don't stop till k 5.5. Expect a fifth reduction in dialysis and more than a fifth reduction in heart failure on top of ace or arb. Key is to start when egfr preserved to mitigate risk of hyperK
